The start of a new school year comes with plenty of things for families to remember. Here’s one that’s worth checking off your list: complete your child’s forms in the OCSB Parent Portal.
It only takes a few minutes, but these forms help your school have the information and permissions it needs to support your child throughout the year.
Why completing your forms matters
Your Parent Portal forms help us:
- Keep your contact and emergency information up to date
- Make sure your child can participate in local outings and classroom activities
- Understand your choices about how your child’s name, photo, and work may be shared
- Ensure students understand how to use technology safely and responsibly
Some forms need to be completed for each child in your family, so please take a moment to check that you haven’t missed any.
Four forms to check
- Consent for Use and Disclosure: This form lets you choose how your child’s name, photo, and work may be used in places such as class photos, yearbooks, school and OCSB websites, and social media. Failure to submit this form will be interpreted as consent to publish your child’s name and photograph in class photos and yearbook only.
- Off-Site Permission Form: A quick walk to the park? A visit to a nearby splash pad? This form gives your child permission to participate in supervised local outings without requiring a separate permission form each time. You’ll still receive individual permission forms for larger field trips.
- Responsible Use of Technology Form: Technology is an important part of learning. This agreement helps students understand their responsibility to use school and personal devices safely, respectfully, and appropriately.
- Verification Form: Take a moment to make sure your contact information and emergency contacts are correct. You only need to submit this form if something has changed.
New to the Parent Portal?
The OCSB Parent Portal is your one-stop location for important information about your child. Along with completing school forms, you can use it to access report cards, IEPs, high school credit progress and class schedules.
If you don’t already have an account, visit the Parent Portal page on the OCSB website for instructions on how to register and activate your account.
When signing in, remember to use the same email address you have on file with your child’s school. If you’re having trouble accessing your account, your school office can help.
