Welcome to a new school year in the Ottawa Catholic School Board (OCSB)! September is a busy and exciting time, filled with new beginnings and important updates. One key way we work together to support your child’s success is by ensuring we have the most up-to-date information, which starts with completing your online forms in the OCSB Parent Portal.
These forms are essential. They help us stay connected, keep your child safe, and ensure they have access to the full range of learning experiences we offer.
Why are these Forms Important?
Completing your Parent Portal forms ensures:
- We have your most current contact and emergency information.
- Your child can take part in local field trips and classroom activities.
- You’ve shared your preferences for how we use and share your child’s name, work, or photo.
- Your child understands expectations for using technology responsibly.
- Without your completed forms, your child may miss out on important opportunities like neighbourhood outings or access to school technology.
- Please log into your Parent Portal account and complete these forms for each of your children.
Creating a Parent Portal Account
If you do not have a Parent Portal account, go to the Parent Portal page on our Board website and follow the instructions to register and activate your account. In addition to submitting school forms, you can use the Parent Portal to view your child’s report cards, IEP, high school credit progress, and class schedule.
Forms to Complete Now
Please take a few moments to log in to the Parent Portal and complete the following important forms. (You’ll find them in the left-hand navigation menu of your Parent Portal dashboard. Click on each form to open it, complete the necessary fields, and submit when finished.)
- Consent for Use and Disclosure: This form is essential for various school operations and communication. It tells us whether we have permission to use your child’s name, work and photo in digital and physical spaces, including class photos, yearbooks, and social media. The form allows you to choose between providing consent for everything, declining consent for everything, or only allowing consent for your child’s name and photo in class photos and yearbooks. Failure to submit this form will be interpreted as consent to publish your child’s name and photograph in class photos and yearbook only.
- Off-Site Permission Form: This form gives permission for your child to go on supervised short trips, like visits to local parks or splash pads. This means you won’t need to sign a separate permission slip each time for these smaller outings. (Please note: You will still receive separate permission slips for larger field trips.)
- Responsible Use of Technology Form: This form is an agreement that helps your child understand how to use technology (school devices or personal ones) safely and respectfully.
- Verification Form: This form ensures we have the correct contact information for you and your child’s emergency contacts. Please review your contacts in the Parent Portal annually, but only complete this form if changes are needed.
